Defining career paths in health systems improvement

ScienceDaily (Nov. 29, 2012) ? The sheer number of efforts aimed at improving the quality and efficiency of the U.S. health care system -- ranging from portions of the national Affordable Care Act to local programs at individual hospitals and practices -- reflects the urgency and importance of the task. One aspect that has received inadequate attention, according to three physicians writing in the January 2013 issue of Academic Medicine, is training the next generation of experts needed to help lead these efforts. In their Perspective article, which has been released online, the authors propose a framework for career development in what they call "health systems improvement," a term that encompasses a broad range of activities -- including management, research and public policy -- to improve the quality and efficiency of our systems of care.

"My co-authors and I each had experience in nonclinical fields such as government, consulting or law before entering medical school, leading to countless conversations with our classmates and co-residents about atypical career paths," explains Clay Ackerly, MD, MSc, management and policy fellow at Massachusetts General Hospital and the Massachusetts General Physicians Organization, a co-author of the Academic Medicine article. "During these conversations most trainees echoed common themes of wanting to have careers that allow them to improve the quality of our care delivery systems. However, most of them also had difficulty articulating their exact interests, and it became clear that we lacked a common lexicon to discuss their specific interests and potential career opportunities."

The authors -- also including Ami Parekh, MD, JD, medical director of Health Systems and Innovation at the University of California, San Francisco, and Daniel Stein, MD, senior medical resident at Brigham and Woman's Hospital -- write that while a few academic medical centers have developed programs addressing health systems improvement, many medical trainees "may become overwhelmed by the sheer number of, and lack of clarity among, possible career paths." Since many of today's health care leaders reached their current positions through what the authors call "circuitous and often serendipitous career paths," their ability to guide and advise young physicians may be limited.

Based on a series of conversations with medical students, residents and leaders in health systems improvement, the authors put together their framework. It starts with three core focus areas -- research, policy and management -- defines three intersections -- policy advising, policy translation, and implementation science -- and includes examples of potential careers, such as academic administration, health services research and government relations.

"The framework is largely a collection of definitions -- a new taxonomy, if you will -- and while some may disagree with the dividing lines, we wanted to put some structure around the passion so many trainees feel for improving the care of patients," Ackerly explains. "We hope this framework can help anyone with that passion -- from any background, not just MDs -- find productive ways to develop their interest and have an impact."

Ackerly and his co-authors suggest that trainees interested in careers in health systems improvement take a few simple steps to help guide their early career development. Some are as basic as reflecting on their own preferences, including whether they want to practice clinically, and identifying one core area on which to focus, as well as one to deemphasize. Trainees may also consider the possibility of additional formal training -- such as research fellowships or advanced degrees in public health, business, health administration or public policy. Noting that their proposed framework needs validation, the authors plan to monitor how it is used by both trainees and their mentors, refine their recommendations, and expand the examples of career options included in the paper.

"We need to bring as much talent as possible into this field. But until we have better defined career paths for trainees who want to engage in this work, many of them may become, at best, confused and, at worst, demoralized," Ackerly stresses. "We hope that making those career paths more explicit will lead to an energized workforce and will attract the needed institutional investments in time and resources. The struggle to improve the quality of health care should never end -- patients deserve nothing less."

Meet Jessie Royer

Growing up on a Montana cattle ranch, Jessie Royer had a love for animals from an early age. She rode horses for as long as she can remember with her pet border collie close behind. From about the time she was 10 yrs old she would hook up her border collie ?Bear? with a horse halter as a harness and he would pull firewood to the house and hay to the goats.

Jessie?s first team was her dog Bear and her billy goat named Billybob. At the age of 15 she saw her first ?real? sled dogs at a race close by. Some of the mushers took her on rides and she was hooked from then on.

Jessie got her first sled dog named Nelson a couple weeks later and ended up going to those same races a year later with her own 11 dog team at the age of 16.

She started racing and at 17 she became the first female and youngest person to ever win Montana?s Race to the Sky 500 mile race.

Running in races like Minnesota?s John Beargrease 500 mile marathon, Wyoming Stage Stop, and Canada?s Northern Lights Challenge, it wasn?t long before moving to Alaska to train and race was next on the horizon. Living in Alaska for 14 yrs, she has competed in almost every big race including the Iditarod.

In 2001 Jessie finished her first Iditarod in 14th place earning her the ?Rookie of the Year Award?. For the next 10 yrs she would finish this 1000 mile race in the top 20 and even top 10 three times. For the years 2008, 09, 10, and 2011 she was the highest placing female. In 2009 Jessie?s lead dog ?Kuling? received the Golden Harness Award for being the outstanding lead dog.

Kuling went on to finish 9 Iditarods on Jessie?s team and even completed her last one at the age of 12 on a 10th place team before retiring to the coach.

Currently Jessie is living back in Montana working with Triple Creek Guest Ranch in Darby, MT giving rides to guests and teaching them how to drive their own team. She will also be training a team to run in the 2013 Iditarod as well. With a kennel of 60 dogs to train and care for, this is certainly a lifestyle not just a job. It is a labor of love! Jessie still enjoys her horses as well and loves to work cows and go on pack trips in the mountains with them. She also enjoys hunting and competing in mounted shooting events with her horses. Basically, if it has anything to do with dogs, horses, or guns you will probably find Jessie trying her hand at it. With a big love for the great outdoors, there just is simply no better way to travel and see the countryside than on the back of a dogsled or a horse!

Jon Gruden offered NFL ownership to become Tennessee's next football coach?

They call the college football coaching carousel the ?crazy season,? and they call it that with good reason: From the time the first coach is fired, straight up until the last one is hired, college football basically turns into one big high school cafeteria. Rumors all of a sudden become fact, innuendo becomes iron clad, and the only person?s word you can trust is? well, no one?s actually.

Still, as crazy as it gets, there is still the occasional rumor that is so far-fetched, so stunning, so gosh darn hard to believe, that everyone agrees on one thing: ?There?s no way that can be true.? ?

One of those rumors popped right after midnight on the East Coast Wednesday, involving the ex-NFL head coach of everyone?s dreams, Jon Gruden. It seems like just about every college program with an opening right now has their eye on Gruden, and each seems willing to top the last to get him.

Apparently though, the University of Tennessee has gone above and beyond everyone else, and has and not only offered Coach Chuckie the standard fare of cash, cars and women (well at least the first two, anyway), but also?

?are you ready for this?...

? Part ownership in an NFL team!!!!

It seems too crazy to be true, until you remember that one of Tennessee?s biggest boosters is a guy by the name of Jim Haslam, who also happens to be the owner of the Cleveland Browns. And apparently he wants Gruden so bad, he?s even willing to hand over a piece of Brown pie to get him.

The news was first reported by News Channel 3 in Memphis. Here?s their report:

Knoxville, TN-News Channel 3 has learned former NFL coach Jon Gruden has a contract offer in hand from the University of Tennessee and a decision could be made as early as Wednesday. Part of the offer is Gruden getting a piece of the NFL?s Cleveland Browns, who were recently bought by Jim Haslam, one of UT?s biggest boosters who played on the Vols? national championship teams in the early fifties. He?s also the father of Tennessee governor Bill Haslam. A potential hangup could be over money Gruden may be owed by the Tampa Bay Buccaneers from his stint as head coach there that ended in 2008. The NFL is looking into the conflict of owning part of one team while getting money from another. Gruden is currently working as an NFL analyst for ESPN. He started his coaching career in 1986 as a graduate assistant at Tennessee where he met his wife, a former UT cheerleader. Gruden?s two head coaching stints in the NFL were with Oakland and Tampa Bay. He coached the Bucs to a Super Bowl win in the 2002 season.

Again, this all seems to be too crazy to be true, until you start putting the puzzle pieces together.

Since it became clear a few weeks ago that Derek Dooley would no longer be back at Tennessee, Haslam?s name has come up as one of the big kingmakers in Knoxville. Haslam- whose son is actually the governor of Tennessee- might not have the clout to name the next head coach, but he did have the cash to lure a big name, such as Gruden?s to town.

Now we?ll see if Haslam is able to put his money where his mouth is.

If the report is true, Gruden should make some sort of final decision on Tennessee by Wednesday.

Our hunch?

Gruden is a lot more likely to be in the ESPN booth next season calling Browns games, then in the ownership box watching them.

The NFL?s best team are beginning to separate themselves from the pack as the Power Rankings head into their 13th week. The top five teams remain unchanged, and only one top 10 team lost this past weekend as the race for playoff positioning intensifies.

That one loss of course, was suffered at the hands of the New York Giants, who shut down Aaron Rodgers and the Green Bay Packers for the second time in the past 11 months. New York?s win temporarily halted all the talk of their regular midseason swoon.

The wild card races in both conference continue to be anyone?s guess as six teams that began last week with serious playoff aspirations suffered losses, some of them in embarrassing fashion (yes we?re talking to you Dallas, Pittsburgh, Minnesota and Seattle). Even fringe teams like the New York Jets and Tennessee Titans failed to play like their postseason lives depended on it.

Although the cream seem to be rising to the top, there is still no obvious Super Bowl favorite, let alone conference champions, among the playoff hopefuls. That should make for an interesting and exciting final five weeks of the NFL season.

  1. Houston Texans (10-1): The Texans found a way to pull out a second consecutive overtime game against a desperate Detroit Lions team.? Houston can improve to 4-0 in the AFC South with a win at Tennessee this weekend.
  2. San Francisco 49ers (8-2-1): Colin Kaepernick passed his second tough test in as many weeks in leading the 49ers to 31-21 road win over the New Orleans Saints. Of course the two defensive touchdowns helped his cause.?
  3. Atlanta Falcons (10-1): Atlanta delivered one of their best performances of the year with their 24-23 victory over the Buccaneers in Tampa Bay. The Falcons can clinch a share of the NFC South title with a Thursday night home win over the Saints coupled with a Tampa Bay loss at Denver on Sunday.
  4. Baltimore Ravens (9-2): The Ravens should send the entire Chargers defense some very nice Christmas gifts after Ray Rice?s 4th-and-29 conversion paved the way for an overtime victory at San Diego.
  5. New England Patriots (8-3): After embarrassing the New York Jets on Thanksgiving night, the Patriots can clinch the AFC East with a road win over the Miami Dolphins on Sunday.
  6. New York Giants (7-4): When the Giants play like they did in Sunday night?s demolition of the Green Bay Packers, it?s easy to see why they?re the defending Super Bowl champions.
  7. Denver Broncos (8-3): Knowshon Moreno filled in admirably for the injured Willis McGahee as the Broncos pulled out a tougher-than-expected win over the Chiefs at Arrowhead Stadium. Denver can also wrap up the AFC West this weekend with a home win over the Bucs coupled with a Chargers loss to the Bengals.
  8. Chicago Bears (8-3): The Bears were happy to have Jay Cutler?and their real defense?back in a 28-10 victory over the Minnesota Vikings. The Packers loss has put Chicago back in the driver?s seat in the NFC North division race. ?
  9. Green Bay Packers (7-4): The Packers have to be questioning themselves after another blowout loss to the Giants. They now find themselves back in the wild card hunt and are stil waiting to get some key players back from injury.
  10. Cincinnati Bengals (6-5): The Bengals have won three in a row thanks to a soft schedule and much improved play from quarterback Andy Dalton. If Benjarvis Green-Ellis can continue to run the ball effectively, another wild card playoff berth is within reach.
  11. Tampa Bay Buccaneers (6-5): The Bucs can?t afford to sulk after a soul-deflating home loss to the Falcons. Tampa Bay will fall further into the muddied NFC playoff picture if they lose to the Broncos in Denver on Sunday.
  12. Indianapolis Colts (7-4): Credit the Colts? defense for holding a very good Buffalo offense to only 13 points. The remaining schedule is tough, but Indianapolis controls it?s own destiny in the battle for the AFC?s top wild card spot. If the playoff started today, that would mean a first-round date with Peyton Manning and the Denver Broncos. You can?t make this stuff up.
  13. Pittsburgh Steelers (6-5): Even with a third-string quarterback, eight turnovers against the Cleveland Browns is unacceptable. Big Ben can?t get back fast enough.
  14. Seattle Seahawks (6-5): Losing at Miami to fall to 1-5 on the road was bad enough for the Seahawks. Losing their two starting corners for the rest of the season due to drug suspensions would be a death sentence for Seattle?s playoff hopes.
  15. Minnesota Vikings (6-5): The 28-10 loss at Chicago was tough for the Vikings. Losses by Dallas, Tampa Bay and Seattle cushioned the blow quite a bit, but another loss at Green Bay this weekend will likely leave Minnesota in the middle of a brutal battle for the NFC?s final wild card spot.
  16. Washington Redskins (5-6): RGIII and the Redskins passed a big test by beating the Cowboys in Dallas on Thanksgiving Day. A home win over the Giants would put them right back in the hunt for the NFC East and give Griffin III?s MVP candidacy a major boost.?
  17. New Orleans Saints (5-6): Two pick-sixes by Drew Brees killed New Orleans? chances of upsetting the 49ers and moving above .500 for the first time all season. The Saints will need to move on quickly with a season-defining visit to Atlanta on tap Thursday night.
  18. Dallas Cowboys (5-6): The slow starts and baffling interceptions are killing the Cowboys at a time when Dez Bryant has become their most reliable player on offense. The mounting injuries on defense aren?t helping after Dallas lost two more starters two potentially season-ending injuries in Thursday?s loss to the Washington Redskins.
  19. Miami Dolphins (5-6): Kudos to the Dolphins for regrouping from back-to-back conference losses and winning a crucial home against a Seahawks team that needed a victory just as much as Miami did. They face a tough challenge building on that momentum with the Patriots coming to town.
  20. Detroit Lions (4-7): It?s hard to give Ndamukong Suh the benefit of the doubt given his checkered history. However, it?s hard to see how he could even see where his leg was going on the play that nearly earned him another suspension. ? ? ? ? ?
  21. Tennessee Titans (4-7): The Titans have to be the most difficult team in the NFL to figure out. They destroy the Dolphins 37-3 in Miami in Week 10, only to return from their bye week and lose a close game at Jacksonville with their playoff lives at stake.
  22. New York Jets (4-7): Regardless of how soft their closing schedule is, it?s difficult to think of the Nets as a playoff team, even if they do win out, after the way they played against New England on Thanksgiving night.
  23. Buffalo Bills (4-7): Where was Buffalo?s offense?apart from C.J. Spiller?during Sunday?s loss at Indianapolis. The Bills squandered a strong defensive effort in a difficult road environment.
  24. St. Louis Rams (4-6-1): Although the Cardinals are reeling the Rams were able to get some big turnovers in sweeping the season series. They?ll need to duplicate that effort if they hope to challenge the 49ers the way they did in the Week 10 tie at San Francisco.
  25. San Diego Chargers (4-7): It?s hard to pin Sunday?s inexplicable loss on Norv Turner, but giving up a 4th-and-29 play is indicative of the type of play that has plagued San Diego during this 1-6 slide.
  26. Cleveland Browns (3-8): The Browns are tied for 7th in the NFL with a plus-eight turnover ratio after forcing the same number in a home win over the Steelers. Cleveland, at 2-3, now owns a better division record that Pittsburgh (1-2) and Cincinnati (1-3), and could earn it?s first back-to-back wins of the year with a victory at Oakland.
  27. Carolina Panthers (3-8): Even if it took more than half the season, it was good to see Cam Newton look more like the player that showed such promise after a record-setting rookie season.
  28. Arizona Cardinals (4-7): Ryan Lindley looked very much like a player making his first NFL start, throwing four interceptions in a 31-17 loss to St. Louis. Hard to imagine that a team that began the year 4-0 may not win another game.
  29. Oakland Raiders (3-8): Carson Palmer picked a bad time to have his worst game of the year as the Raiders were crushed in his first game at Cincinnati since being traded last season. Oakland hopes to have Darren McFadden back against a Browns defense that is playing extremely well lately.
  30. Philadelphia Eagles (3-8): You know the wheels have come off in Philadelphia when even a surprisingly positive performance from running back Bryce Brown (178 yards and two touchdowns) was marred by two third-quarter fumbles.?
  31. Jacksonville Jaguars (2-9): Chad Henne continued to make a world of difference for Jacksonville?s offense, leading the Jaguars to a 24-19 victory over the Titans. He shouldn?t have much trouble delivering another solid performance agains Buffalo?s 18th-ranked pass defense.
  32. Kansas City Chiefs (1-10): The Chiefs defense had their second impressive showing in the past three weeks, holding Peyton Manning and the Broncos to 17 points. Of course the Brady Quinn-led offense could only must nine points, so there?s that.

Study leaves women with conflicting advice on mammograms

CHICAGO (Reuters) - Controversial U.S. guidelines for mammography issued in 2009, calling for screening every two years rather than annually for women over 50 years old, can result in breast cancers being missed, according to U.S. researchers studying the hotly debated topic.

This latest study assessing the impact of revised guidelines issued by the U.S. Preventive Services Task Force three years ago comes on the back of a chilling analysis published last week in the New England Journal of Medicine (NEJM).

The NEJM study, using three decades of U.S. government data, found that nearly one in three patients - or 1.3 million women - whose cancer was detected through routine mammograms was treated for a tumor that may not have been life threatening.

Routine screening has traditionally been recommended by the task force and the American Cancer Society for all women over the age of 40. The American Cancer Society has not changed its stance, but the task force issued new guidelines in 2009, recommending mammography every two years for women 50 to 74 years old.

Dr. Elizabeth Arleo of Weill Medical College of Cornell University and lead researcher examined the impact of the revised task force guidelines on women 40 to 49 years of age and 65 years and older.

Arleo analyzed data on screening mammography at New York Presbyterian Hospital/Weill Cornell Medical Center between 2007 and 2010. Over those four years, 43,351 mammograms were performed, leading to the detection of 205 breast cancers. Nearly 20 percent of cancers detected with screening mammography were found in women in their 40s.

"In our book, it seems unacceptable to miss 19 percent of breast cancers, half of which were invasive," she said, noting that "invasive" cancers are more advanced because they reside in the breast tissues and not just the breast ducts.

"Our findings favor the American Cancer Society recommendations. Women over 40 should have annual mammograms. In my book, there's no confusion. I tell my patients, I tell my friends, and I tell my mother to get annual mammograms," Arleo said in a telephone interview. "We hope we can quell some of the confusion."

She presented her findings at the Radiological Society of North America meeting in Chicago.

Arleo dismissed the NEJM analysis, calling the premise and conclusions of the analysis flawed, mostly because no one can say for sure which cancers would have progressed to an advanced or life-threatening state.

"We need to understand the biology of cancer so we know which cancers to treat. Until that question is answered, screening is our best shot," she said.

She acknowledged that false positives are a problem of widespread screening.

"But for every test, you have to do a risk-benefit analysis, not just for mammography," Arleo added.

Before joining Life Time, Steve Coungeris was overweight all of his life. Although he lost weight throughout the early ?80s and ?90s, the results didn?t stick. ?I was definitely not happy with myself,? he says, ?I tried but was never able to repeat and maintain those losses of 15 and 25 pounds.?

Steve?s son Nick was born in 1993, and Steve?s life seemed complete?for the most part. ?I had a wonderful wife, beautiful son and great job,? he says. ?I was happy with everything in my life except for my weight. It was always on my mind, but I was never motivated enough to do what was required.?

In 2005, Steve?s wife was diagnosed with breast cancer. After chemotherapy and radiation treatment, her health seemed to improve at the end of the year. By April of the following year, the cancer returned and spread to her bones, lungs and liver. She passed away in July 2006. ?Obviously, my weight was not high on my list of importance at that time or for the next three years,? Steve says.

As Steve?s focus shifted to getting Nick through school, his weight reached 295 pounds. Horrified at the thought of weighing more than 300 pounds, Steve decided it was time to ditch the excess pounds for good. In 2009, he joined .

After taking fitness tests like and assessments, Steve worked with a dietician and started sessions with Jason Miller. From September 2009 to February 2010, Steve met with Jason three days a week. He lost 20 pounds and gained a better understanding of exercise and nutrition. He also recovered from an Achilles issue that had prevented him from using a treadmill.

Despite his progress, Steve was apprehensive about maintaining his health after Nick left for college. ?I knew it was going to be my time, but I still needed consistency, accountability and support,? he says. ?I decided only I could stop me, so I set milestone weight-loss goals in increments of 10.?

Steve set a goal weight of 240 pounds, and Jason recommended that he join with MCA Coordinator and Personal Trainer Ross Harrison to meet it. Fellow TEAM members and Ross have supported Steve throughout the training. ?Ross is incredible,? Steve says. ?He pushes us to our limits and then asks us for a little bit more, but always with great instruction and guidance.?

In addition to helping Steve step up his training, Ross introduced him to MCA classes. Steve started and classes twice a week with Ross and Virgil Strzelecki, Life Time Fitness Bloomingdale Assistant Personal Training Department Head.

Israeli PM Netanyahu suddenly seems vulnerable

JERUSALEM (AP) ? Israeli Prime Minister Benjamin Netanyahu, who appeared to be cruising to re-election a few weeks ago, suddenly appears vulnerable as the country prepares to go to the polls in January.

The political comeback of a popular former foreign minister on Tuesday, coupled with the ruling Likud Party's selection of an especially hard-line slate of candidates, has suddenly raised questions about Netanyahu's prospects. Eager to portray Netanyahu as an extremist, opposition parties see an opportunity to mount a formidable challenge to the Israeli leader.

Ousting Netanyahu remains a formidable task, but the return of Tzipi Livni, who served as Israel's foreign minister and chief peace negotiator from 2006 to 2009, injected a high-profile name into what had been a lackluster race. Well respected internationally, Livni immediately took aim at what she called a "leadership vacuum" and promised an aggressive push for peace with the Palestinians.

"I came to fight for peace," she said. "And I won't allow anyone to turn peace into a bad word."

During Netanyahu's nearly four years in office, peace efforts with the Palestinians have remained frozen.

The Palestinians refuse to resume talks unless Israel stops building settlements in the West Bank and east Jerusalem, occupied areas that the Palestinians claim for an independent state.

Netanyahu blames the Palestinians for the deadlock. The prime minister imposed a partial freeze on settlement construction early in his term, leading to a brief attempt to resume negotiations. But Netanyahu refused to extend the construction slowdown, and the talks quickly collapsed. He says talks should resume without any preconditions.

The deadlock has pushed Palestinian President Mahmoud Abbas to head to the United Nations this week to upgrade the Palestinian observer mission to "nonmember state" status. The General Assembly is expected to pass the request.

The Palestinians believe the international endorsement, while largely symbolic, will bolster their position if negotiations resume. Israel, backed by the United States, opposes the bid and has been furiously lobbying allies to oppose it, saying all differences must be resolved through negotiations. In a setback for Netanyahu, France announced Tuesday that it would vote in favor of the Palestinians.

At a time when there are no talks with the Western-backed government of Abbas, Israel this week opened an indirect dialogue with Abbas' rival, the Islamic militant Hamas movement, as part of a cease-fire that ended an Israeli military offensive in the Gaza Strip last week. Israel considers Hamas, which seized control of Gaza five years ago, a terrorist group.

"Everything is upside down: a government that negotiates with terrorists and freezes all dialogue with those who work to prevent attacks," Livni said.

Livni told the audience that the fighting in Gaza, in which her youngest son was mobilized to Israel's southern front, had factored into her decision to return to politics.

"A week ago when my youngest son, today an officer in the Paratroopers, went down south, I sent him a text message that I had decided to fight on my turf, politics, so that he maybe won't have to fight on his turf, the battlefield," she said.

Throughout his term, Netanyahu relied on a handful of moderate figures in his Cabinet to blunt international opposition. Following a pair of events this week, Netanyahu appears to have lost this political cover.

On Monday, Defense Minister Ehud Barak resigned from politics. Barak, a former prime minister, had often served as Netanyahu's unofficial envoy to Washington to ease tensions with the White House.

Then Monday night, Netanyahu's Likud Party announced its slate of candidates for the Jan. 22 election.

The list was dominated by hard-line supporters of West Bank settlements. Some candidates have alienated mainstream Israelis with failed attempts in parliament to stifle dissent and rein in a Supreme Court that they believe is too independent. In addition, several prominent moderates were effectively ousted. Netanyahu's decision to join forces with the ultranationalist party headed by his foreign minister, Avigdor Lieberman, has added to concerns that his next coalition will be too hard line for many.

Menachem Hofnung, a Hebrew University political scientist, said the Likud could have a hard time appealing to centrist voters with its new list. "After seeing the Likud's results, the race is (wide) open," he said.

Livni, 54, joins a field that includes the centrist Labor Party, led by former journalist Shelly Yachimovich, and the centrist "Yesh Atid," led by former anchorman Yair Lapid.

While largely similar in ideology, these rival parties have focused largely on domestic economic issues. Livni made clear her new party, called "The Movement," would focus on resuming peace efforts.

"I decided to give an answer to people who don't have anyone to vote for," Livni said. "This party will return this hope that was lost."

It won't be an easy task. Recent polls have predicted Likud would be the largest party in the 120-seat parliament and in a strong position to cobble together a majority coalition. The centrist parties remain divided, and their leaders have been reluctant to join forces.

A survey published Tuesday predicted that Livni's new party would garner just nine seats, Labor would win 20 and Lapid's Yesh Atid would get only five. That would leave the centrist bloc far short of the 61 seats needed to form a coalition.

In contrast, Netanyahu's Likud would win 37 seats, making it by far the largest single party in parliament, with hard-line nationalist and religious parties giving it a majority.

The poll by the Maagar Mochot agency surveyed 504 people on Sunday and Monday and had a margin of error of 4.5 percentage points. However, the poll was conducted before Livni's comeback and Likud primary results were announced, so the numbers are expected to change.

Livni herself is the daughter of one of Likud's founding fathers and entered politics in 1999 as a Likud lawmaker.

But like many other former hard-liners, she has moved over to Israel's dovish left to confront what many believe to be a demographic time bomb. If Israel continues to control millions of Palestinians, they say, it will cease to exist as a democracy with a Jewish majority. The solution, they say, is establishing an independent Palestinian state alongside Israel.

As foreign minister, Livni forged a strong relationship with her American counterpart, Condoleezza Rice, as well as the Palestinians. The sides claimed to have made great progress.

Although her legacy was tarnished by criticism over an Israeli military offensive in Gaza in 2009 that left hundreds of civilians dead, she nonetheless remains popular internationally. She has been identified by Time, Newsweek and Forbes magazines as one of the world's most influential women.

But she has stumbled as a politician. Livni led centrist Kadima Party in 2009 elections after then-Prime Minister Ehud Olmert was forced to resign because of a corruption scandal.

While Kadima won the most parliamentary seats in that election, Livni was unable to form a coalition and confined to the opposition. Early this year, she was ousted as party leader and left politics. Kadima has steadily lost support since then, and recent polls have forecast Kadima may not win even a single seat in parliament.

ROME (AP) -Napoli moved back into title contention with a 1-0 win at Cagliari on Monday, placing the southern club two points behind Italian league leader Juventus.

With Napoli's leading scorer Edinson Cavani suspended and fellow forward Goran Pandev out injured, Marek Hamsik found the target in the 72nd minute with an angled shot from inside the area to finish off an action began by Lorenzo Insigne, who also hit the post in the first half.

Meanwhile, Inter Milan wasted a chance to move within one point of the lead with a 1-0 loss at Parma, where Nicola Sansone put the hosts in front with an impressive solo action in the 75th.

ROYAL OAK (WWJ) ? A debate is heating up in downtown Royal Oak over whether businesses should be allowed to use rolling gates over doors and windows for security purposes.

A jewelry shop and deli recently installed the gates, but city officials are trying to have them removed.

Royal Oak Commissioner David Poulton is one of the people opposed to the security measure.

?Downtown Royal Oak is a?walk-able?community,? said Poulton. ?We have concerns about these security curtains and the image they send, to me, it sends that it?s a high crime district and that is not what it is.?

Poulton is part of a city-led effort to pass an ordinance that would ban the rolling curtains.

Infinity Jewelers and Franklin Deli, the companies that put them up, could not be reached for comment.

FDA flexes new muscles, shuts down plant

WASHINGTON (AP) ? The Food and Drug Administration halted operations of the country's largest organic peanut butter processor Monday, cracking down on salmonella poisoning for the first time with new enforcement authority the agency gained in a 2011 food safety law.

FDA officials found salmonella all over Sunland Inc.'s New Mexico processing plant after 41 people in 20 states, most of them children, were sickened by peanut butter manufactured at the plant in Portales and sold by Trader Joe's grocery chain. The FDA suspended Sunland's registration Monday, preventing the company from producing or distributing any food.

The food safety law gave the FDA authority to suspend a company's registration when food manufactured or held there has a "reasonable probability" of causing serious health problems or death. Before the food safety law was enacted early last year, the FDA would have had to go to court to suspend a company's registration.

Michael Taylor, the FDA's deputy commissioner for foods, said the agency's ability to suspend a registration like this one is a major step forward for the agency.

"Consumers can be assured that products will not leave this facility until we determine they have implemented preventive measures that are effective to produce safe products," Taylor said.

Sunland had voluntarily closed its plant after a September outbreak and planned to reopen its peanut processing facility on Tuesday, with hopes of selling peanut butter again by the end of the year. Sunland's Katalin Coburn said FDA's decision to suspend the registration was a surprise to the company and Sunland officials had assumed they were allowed to resume operations.

The company now has the right to a hearing and must prove to the agency that its facilities are clean enough to reopen. Coburn said Sunland is cooperating with FDA and company officials hope they can be up and running again soon.

Sunland is the nation's largest organic peanut butter processor, though it also produces many non-organic products. The company recalled hundreds of organic and non-organic nut butters and nuts manufactured since 2010 after Trader Joe's Valencia Creamy Peanut Butter was linked to the salmonella illnesses in September.

In addition to Trader Joe's, Sunland sold hundreds of different peanut products to Whole Foods, Safeway, Target and other large grocery chains.

During a month-long investigation, after the outbreak linked to processor Sunland and to Trader Joe's, FDA inspectors found samples of salmonella in 28 different locations in the plant, in 13 nut butter samples and in one sample of raw peanuts.

The agency also found improper handling of the products, unclean equipment and uncovered trailers of peanuts outside the facility that were exposed to rain and birds.

The FDA said that over the past three years, the company shipped products even though portions of their lots, or daily production runs, tested positive for salmonella in internal tests. The agency also found that the internal tests failed to find salmonella when it was present.

FDA inspectors found many of the same problems ? including employees putting their bare fingers in empty jars before they were filled, open bags of ingredients, unclean equipment, and many other violations ? in a 2007 inspection. Similar problems were recorded by inspectors in 2009, 2010 and 2011, though government officials didn't take any action or release the results of those inspections until after the illnesses were discovered this year.

In a statement issued earlier this month, Sunland's president and chief executive officer, Jimmie Shearer, denied the company knowingly shipped tainted products.

"At no time in its 24-year history has Sunland, Inc. released for distribution any products that it knew to be potentially contaminated with harmful microorganisms," Shearer said in a statement posted on the company's website. "In every instance where test results indicated the presence of a contaminant, the implicated product was destroyed and not released for distribution."

A separate peanut butter outbreak in 2009 not related to Sunland was linked to hundreds of illnesses and nine deaths.

WASHINGTON (AP) ? White House economists warn that the uncertainty of a potential hike in taxes next year for middle class taxpayers under the looming fiscal cliff could hurt consumer confidence during the crucial holiday shopping season.

In a report Monday that coincides with Congress' return after the Thanksgiving holiday, a White House report says that if lawmakers don't halt the automatic increase in taxes for households earning less than $250,000 in the coming days, consumers might even curtail their shopping during the current holiday season.

The report by President Barack Obama's National Economic Council and his Council of Economic Advisers also says a sudden increase in taxes for middle-income taxpayers would reduce consumer spending in 2013 by nearly $200 billion. They say that would significantly slow the economic recovery.

Reading, writing and playing games may help aging brains stay healthy

ScienceDaily (Nov. 25, 2012) ? Mental activities like reading and writing can preserve structural integrity in the brains of older people, according to a new study presented November 25 at the annual meeting of the Radiological Society of North America (RSNA).

While previous research has shown an association between late-life cognitive activity and better mental acuity, the new study from Konstantinos Arfanakis, Ph.D., and colleagues from Rush University Medical Center and Illinois Institute of Technology in Chicago studied what effect late-life cognitive activity might have on the brain's white matter, which is composed of nerve fibers, or axons, that transmit information throughout the brain.

"Reading the newspaper, writing letters, visiting a library, attending a play or playing games, such as chess or checkers, are all simple activities that can contribute to a healthier brain," Dr. Arfanakis said.

The researchers used a magnetic resonance imaging (MRI) method known as diffusion tensor imaging (DTI) to generate data on diffusion anisotropy, a measure of how water molecules move through the brain. In white matter, diffusion anisotropy exploits the fact that water moves more easily in a direction parallel to the brain's axons, and less easily perpendicular to the axons, because it is impeded by structures such as axonal membranes and myelin. "This difference in the diffusion rates along different directions increases diffusion anisotropy values," Dr. Arfanakis said. "Diffusion anisotropy is higher when more diffusion is happening in one direction compared to others."

The anisotropy values in white matter drop, however, with aging, injury and disease.

"In healthy white matter tissue, water can't move as much in directions perpendicular to the nerve fibers," Dr. Arfanakis said. "But if, for example, you have lower neuronal density or less myelin, then the water has more freedom to move perpendicular to the fibers, so you would have reduced diffusion anisotropy. Lower diffusion anisotropy values are consistent with aging."

The study included 152 elderly participants, mean age 81 years, from the Rush Memory and Aging Project, a large-scale study looking at risk factors for Alzheimer's disease. Participants were without dementia or mild cognitive impairment, based on a detailed clinical evaluation. Researchers asked the participants to rate on a scale of 1 to 5 the frequency with which they participated in a list of mentally engaging activities during the last year. Among the activities were reading newspapers and magazines, writing letters and playing cards and board games.

Participants underwent brain MRI using a 1.5-T scanner within one year of clinical evaluation. The researchers collected anatomical and DTI data and used it to generate diffusion anisotropy maps.

Data analysis revealed significant associations between the frequency of cognitive activity in later life and higher diffusion anisotropy values in the brain.

"Several areas throughout the brain, including regions quite important to cognition, showed higher microstructural integrity with more frequent cognitive activity in late life," said Dr. Arfanakis. "Keeping the brain occupied late in life has positive outcomes."

According to Dr. Arfanakis, diffusion anisotropy drops gradually beginning at around age 30. "Higher diffusion anisotropy in elderly patients who engage in frequent cognitive activity suggests that these people have brain properties similar to those of younger individuals," he said.

The researchers will continue to follow the study participants with an eye toward comparing the diffusion anisotropy results over time.

"In these participants, we've shown an association between late-life cognitive activity and structural integrity, but we haven't shown that one causes the other," Dr. Arfanakis said. "We want to follow the same patients over time to demonstrate a causal link."

On the wings of 10 of 19 shooting from player-of-the-year candidate Doug McDermott, the Creighton University Bluejays held off the Arizona State Sun Devils Saturday night to win the 12th Annual Continental Tire Las Vegas Invitational. It was a coming out party for McDermott, the tourney MVP, who is rarely seen by West Coast coaches, fans, players, and pro scouts.

However, McDermott couldn't do it alone. Ethan Wragge's right corner trey with 2:22 to go -- after ASU pulled to within six (their closest margin of the night) -- was needed to finally darken the Sun Devils' upset hopes.

However, Creighton could have put the game away much earlier. Only eight minutes into the first half, the Bluejays raced to a 27-9 advantage. ASU was reeling. However, Jay turnovers off the ASU press, uncharacteristic Jay fouls and missed threes allowed ASU back into the contest. It was a dogfight the rest of the night.

ASU's speedy guard, Jahii Carson (which sounded at times like Nebraska's "Johnny Carson" over the Orleans Arena loudspeakers), lead all scorers with 30. Carson was only the fourth freshman in ASU history to score 30 in a game.

However, the balance and cohesion that has been the Bluejays' trademark over the past two seasons overcame Carson's singular effort. Seven Bluejays scored seven or more points. Moreover, ASU didn't have a consistent answer to Creighton's crisp ball movement, rebounding (the Bluejays had a 41-30 advantage) or second-chance points (Creighton won that category 14-0). Nor did the Sun Devils have a remedy for McDermott's deft post-ups, which literally and figuratively helped "seal" the victory for the tournament winners.

Of the roughly 3000 fans in attendance at the Orleans Arena, at least 80% looked like they were clad in blue. It didn't hurt that Creighton University alum Michael Gaughan gave out complimentary hotel rooms at his family-owned South Point Hotel, Casino and Spa to any Creighton fan attending the tournament. The Creighton-ASU game was a homecoming of sorts for yours truly. Not only am I graduate of Omaha Creighton Prep high school, but my late father Richard, sister Julie, and brother Rick are all graduates of Creighton University in one form or another. It was nice to see some friendly, if rowdy, faces from back home.

Next up for the Jays are Boise State (4-1) on Wednesday and the always-tough St. Joe's (3-1) on Saturday. And it won't get easier after that, as the Jays enter Missouri Valley Conference play.

However, beating the Big 12's Wisconsin (4-2) and the PAC-12's ASU (4-1) on successive nights on a neutral floor here in Vegas will prove valuable come NCAA tournament time. Though the 6-0 Jays are no longer below the radar, this smart, fast, and extremely well-coached mid-major remains a tough match-up for any opponent from any conference, regardless of setting.

Bahamas helicopter crash kills American - World News - MSNBC.com

NASSAU, Bahamas -- One American was killed and four people were injured when a helicopter crashed near an exclusive resort in the Bahamas on Thursday afternoon, according to local police.

The helicopter went down at Baker's Bay Golf and Ocean Club on Great Guana Cay in the Abacos island chain, according to Superintendent Noel Curry at the Marsh Harbour police station.

The Miami Herald identified one of the survivors as Jeffrey Soffer, head of the Miami-based Turnberry Associates, a leading property firm. Turnberry owns the famed but financially troubled Fontainebleau resort in Miami Beach as well as condominiums in Las Vegas, the Bahamas and Washington D.C.


The newspaper reported that, according to a Soffer family source, the person who died was the helicopter?s owner, a businessman who lived in the Bahamas, and the others on board were an unidentified woman, the pilot and a person who works with Soffer. They were headed to the Bahamas for Thanksgiving. Authorities did not identify any of the victims.

The Herald, citing the family source, said Soffer walked away from crash with bruises but no serious injuries.?The three passengers were flown back to the United States by private jet, Reuters reported.?The Herald reported that the Soffer family source said Soffer was taken to the University of Miami Hospital for evaluation.?

The helicopter was attempting to land when it was buffeted by a wind gust and the pilot lost control, according to the Nassau Guardian newspaper.

Eyewitnesses said the helicopter's rotor blades hit the ground near the Baker's Bay townhomes. The co-pilot was killed by the blades, and the pilot suffered serious injuries.

The Bahamian Minister of Transport and Aviation, Glenys Hanna-Martin, said a full investigation into the crash was being launched.

Baker's Bay is a luxury resort about 150 miles off Florida's southeast coast with an oceanfront golf course and private beach club, according to its website.

Protests after "pharaoh" Mursi assumes powers in Egypt

CAIRO (Reuters) - Egyptian President Mohamed Mursi's decision to assume sweeping powers caused fury amongst his opponents and prompted violent clashes in central Cairo and other cities on Friday.

Police fired tear gas near Cairo's Tahrir Square, heart of the 2011 uprising that toppled Hosni Mubarak, where thousands demanded Mursi quit and accused him of launching a "coup". There were violent protests in Alexandria, Port Said and Suez.

Opponents accused Mursi, who has issued a decree that puts his decisions above legal challenge until a new parliament is elected, of being the new Mubarak and hijacking the revolution.

"The people want to bring down the regime," shouted protesters in Tahrir, echoing a chant used in the uprising that forced Mubarak to step down. "Get out, Mursi," they chanted, along with "Mubarak tell Mursi, jail comes after the throne."

Mursi's aides said the presidential decree was intended to speed up a protracted transition that has been hindered by legal obstacles but Mursi's rivals condemned him as an autocratic pharaoh who wanted to impose his Islamist vision on Egypt.

"I am for all Egyptians. I will not be biased against any son of Egypt," Mursi said on a stage outside the presidential palace, adding that he was working for social and economic stability and the rotation of power.

"Opposition in Egypt does not worry me, but it has to be real and strong," he said, seeking to placate his critics and telling Egyptians that he was committed to the revolution. "Go forward, always forward ... to a new Egypt."

Buoyed by accolades from around the world for mediating a truce between Hamas and Israel in the Gaza Strip, Mursi on Thursday ordered that an Islamist-dominated assembly writing the new constitution could not be dissolved by legal challenges.

"Mursi a 'temporary' dictator," was the headline in the independent daily Al-Masry Al-Youm.

Mursi, an Islamist whose roots are in the Muslim Brotherhood, also gave himself wide powers that allowed him to sack the unpopular general prosecutor and opened the door for a retrial for Mubarak and his aides.

The president's decree aimed to end the logjam and push Egypt, the Arab world's most populous nation, more quickly along its democratic path, the presidential spokesman said.

"President Mursi said we must go out of the bottleneck without breaking the bottle," Yasser Ali told Reuters.

TURBULENCE AND TURMOIL

The president's decree said any decrees he issued while no parliament sat could not be challenged, moves that consolidated his power but look set to polarize Egypt further, threatening more turbulence in a nation at the heart of the Arab Spring.

The turmoil has weighed heavily on Egypt's faltering economy that was thrown a lifeline this week when a preliminary deal was reached with the International Monetary Fund for a $4.8 billion loan. But it also means unpopular economic measures.

In Alexandria, north of Cairo, protesters ransacked an office of the Brotherhood's political party, burning books and chairs in the street. Supporters of Mursi and opponents clashed elsewhere in the city, leaving 12 injured.

A party building was also attacked by stone-throwing protesters in Port Said, and demonstrators in Suez threw petrol bombs that burned banners outside the party building.

Mursi's decree is bound to worry Western allies, particularly the United States, a generous benefactor to Egypt's army, which praised Egypt for its part in bringing Israelis and Palestinians to a ceasefire on Wednesday.

The West may become concerned about measures that, for example, undermine judicial independence. The European Union urged Mursi to respect the democratic process.

"We are very concerned about the possible huge ramifications of this declaration on human rights and the rule of law in Egypt," Rupert Colville, spokesman for the U.N. Human Rights Commissioner Navi Pillay, said at the United Nations in Geneva.

The United States has been concerned about the fate of what was once a close ally under Mubarak, who preserved Egypt's 1979 peace treaty with Israel. The Gaza deal has reassured Washington but the deepening polarization of the nation will be a worry.

"ANOTHER DICTATOR"

"The decree is basically a coup on state institutions and the rule of law that is likely to undermine the revolution and the transition to democracy," said Mervat Ahmed, an independent activist in Tahrir protesting against the decree. "I worry Mursi will be another dictator like the one before him."

Leading liberal Mohamed ElBaradei, who joined other politicians on Thursday night to demand the decree was withdrawn, wrote on his Twitter account that Mursi had "usurped all state powers and appointed himself Egypt's new pharaoh".

Almost two years after Mubarak was toppled and about five months since Mursi took office, propelled to the post by the Muslim Brotherhood, Egypt has no permanent constitution, which must be in place before new parliamentary elections are held.

The last parliament, which sat for the first time earlier this year, was dissolved after a court declared it void. It was dominated by the Brotherhood's political party.

An assembly drawing up the constitution has yet to complete its work. Many liberals, Christians and others have walked out accusing the Islamists who dominate it of ignoring their voices over the extent that Islam should be enshrined in the new state.

Opponents call for the assembly to be scrapped and remade. Mursi's decree protects the existing one and extends the deadline for drafting a document by two months, pushing it back to February, further delaying a new parliamentary election.

Explaining the rationale behind the moves, the presidential spokesman said: "This means ending the period of constitutional instability to arrive at a state with a written constitution, an elected president and parliament."

"THIS IS NOT THE REMEDY"

Analyst Seif El Din Abdel Fatah said the decree targeted the judiciary which had reversed, for example, an earlier Mursi decision to remove the prosecutor.

Mursi, who is now protected by his new decree from judicial reversals, said the judiciary contained honorable men but said he would uncover corrupt elements. He also said he would ensure independence for the judicial, executive and legislative powers.

Although many of Mursi's opponents also opposed the sacked prosecutor, whom they blamed for shortcomings in prosecuting Mubarak and his aides, and also want judicial reform, they say a draconian presidential decree was not the way to do it.

"There was a disease but this is not the remedy," said Hassan Nafaa, a liberal-minded political science professor and activist at Cairo University.

Honest Graft

The book Honest Graft gives us a glimpse into the mind of outspoken, early twentieth century politician George Washington Plunkitt and his views on the pressing issues of the time through his speeches. While few politicians would dare to air their personal observations in public, Plunkitt was blissfully unconcerned with the morally ambiguous nature of his unique opinions, which were delivered as sermons to the masses.

It would be reasonable to assert that there was no issue that Plunkitt felt more strongly about than the act of committing graft. In the book, Plunkitt discerns between two forms of graft: honest graft and dishonest graft. Both forms are illustrated with examples. Plunkitt claims to have never committed dishonest graft in his life but is reasonably proud of his achievements through honest graft on which he states, "I seen my opportunities and I took 'em". The acts which he associates with honest graft include: the usage of information about government developmental works and purchasing property vital to it to sell at a higher price, the sharing of said information with friends and the raising of government salaries.Whereas dishonest graft involved blackmailing criminals and the embezzlement of money from the city treasury. In my personal opinion, the act of graft cannot be justified under modern ethical or legal definitions as acts that Plunkitt considers honest would be considered highly illegal and immoral in modern society.

Plunkitt's position on alcohol consumption is unequivocal and uncomplicated, while he does not believe that liquor should be outlawed he is of the firm belief that politicians cannot e successful if they drink. According to Plunkit, "no matter how well you learn to play the political game, you won't make a lastin' success of it if you're a drinkin' man." (RIORDON, Chapter 19). He substantiates his assertion by giving the example of one of his lieutenants who was a highly capable individual that would require supervision during election time due to his drinking sprees. He further presents the examples of successful Tammany leaders and heads of department that tended to largely abstain from alcohol consumption.

Another issue which seems to incite Plunkitt's passions is the role of party bosses in the primary elections. Plunkitt felt that strong political leadership was required to conduct tumultuous events such as election primaries, without which government would descend into anarchy with various bodies and office bearers being at loggerheads with each other. He felt that this situation would be extremely detrimental to the interests of the city and presented the example of previous administrations without party bosses where: "the heads of departments were at odds all the time with each other, and the Mayor was at odds with the lot of them. They spent so much time in arguin' and makin' grandstand play, that the interests of the city were forgotten." (RIORDON, Chapter 20). He compares this with Tammanny administrations where party bosses were present and exercised control: "Then see how beautiful a Tammany city government runs, with a so-called boss directin' the whole shootin' match! The machinery moves so noiseless that you wouldn't think there was any. If there's any differences of opinion the Tammany leader settles them quietly and his orders go every time." (RIORDON, Chapter 20). Therefore, Plunkitt felt that the presence of party bosses was necessary, in order, to ensure the smooth operation of the political machinery.

The patriotism of Tammanny and its leaders left a deep and indelible mark on Plunkitt's psyche, causing him to proclaim "TAMMANY's the most patriotic organization on earth," (RIORDON, Chapter 17). He qualifies his statement by citing examples of the unwavering diligence with which they observe the 4th of July, Tammany's love of flags and its willingness to volunteer troops for war efforts, comparing Tamanny's behavior to that of rival organizations such as the Citizen's Union and the reformers whom he claims "run off to Newport or the Adirondacks to get out of the way of the noise and everything that reminds them of the glorious day." (RIORDON, Chapter 17). While one isn't given explicit reason to doubt Tamanny's patriotism or love of country, the rationale presented by Plunkitt seems rather superficial and has the potential to be perceived as tongue in cheek by many. Excluding their supposed commitment to war efforts, the majority of acts presented as evidence of patriotism seem artificial and rehearsed, rather than exhibiting any real devotion towards country.

Plunkitt's feelings about the reform movements of the mid to late 1800's and their attempted entry into the political culture were derisive and dismissive and considered their failure to be a direct result of the nature of individuals in these movements that were reformers and not politicians. Plunkitt considered politicians to be individuals that were involved in politics their entire lives and devoted the vast majority of their waking hours to learning the intricacies of the vocation "You have to give nearly all your time and attention to it. Of course, you may have some business or occupation on the side, but the great business of your life must be politics if you want to succeed in it." (RIORDON, Chapter 4). The inability of several reformers to absorb the political culture around them and to adapt and evolve was identified by Plunkitt as the primary reason why reformers were unable to have a lasting impact on politics and many prominent reformers of their times faded away into obscurity for this very reason, while individuals within these movements with political savvy managed to forge long and successful careers
